Introduction : L'épicondylalgie latérale (EL) est un désordre musculo-squelettique fréquent. La place des examens complémentaires n'est pas encore clairement définie. L'objectif de ce travail est donc de proposer et d'évaluer des critères fonctionnels échographiques comme nouvelle approche diagnostique et pronostique de l'EL. Méthodes : Il s'agit d'une étude pilote, observationnelle, prospective et monocentrique. L'évaluation a été réalisée par deux opérateurs en aveugle. La reproductibilité intra et inter opérateur a donc été estimée. Des tests dynamiques ont étudié l'interface entre les muscles long (LERC) et court (CERC) extenseurs radial du carpe et entre les muscles LERC/CERC et le supinateur. Les résultats obtenus ont été corrélés à l'EVA douleur et aux critères fonctionnels évalués par le QuickDash. Résultats : 20 cas et 20 témoins ont été inclus dans l'étude. La reproductibilité de l'échographie fonctionnelle dans l'analyse des mouvements d'ouverture-fermeture de la main a été estimée faible pour les patients et faible à modérée pour les témoins. Un glissement dysharmonieux des fascias entre eux a pu être objectivé du côté pathologique, comme parfois du côté sain également. Conclusion : L'échographie fonctionnelle ne permet pas d'apporter, à ce stade, de critères diagnostiques ni pronostiques de certitude de L'EL. Son défaut de reproductibilité limite sa recommandation. Malgré tout, elle semble pertinente voire prometteuse dans cette indication. L'obtention de critères plus précisément définis, plus facilement mesurables (reproductibilité) et plus spécifiques (autres muscles ou mouvements) sont nécessaires.
